Plastic surgery, whether for cosmetic or reconstructive purposes, requires many choices. The first and most important of these is choosing a reliable surgeon. One of the best indicators of a physician's education is having a Board certification. Ask your surgeon about having a plastic surgery Board certification, as this certification may only indicate a person's eligibility for plastic surgery on the face and all other areas of the body.

A physician must meet the following standards to obtain a board certification:

Being graduated from a certified medical school.
After graduating from medical school, they should have completed at least four years of general surgery training, and after that they have completed at least three years of subspecialty residency in plastic surgery.
Should have passed all relevant written and oral exams.

Members of the Iranian Society of Plastic Surgeons should:

Have a plastic surgery board certification.
Take annual plastic surgery retraining courses.
Perform surgeries in authorized and approved medical centers.

Your surgeon should talk to you about where the surgery will be performed. This can be done at a hospital, outpatient surgery center.

Note: If the operation is performed on an outpatient basis and you are to be discharged after the operation, arrange with someone to take you home and spend the night with you.

Stages of surgery

Step 1 – anesthesia

You will be given medications to make you comfortable during surgery. These medications will cause general anesthesia or relaxation that your surgeon will decide.

Step 2 - cutting

Rhinoplasty is performed either in a closed method in which incisions are made inside the nose or in an open method in which an incision is made in the columella (the middle septum between the nostrils). Through it, the nasal covering is opened and the necessary changes are made in the shape of the nose.

Step 3 - deformation of the structure of the nose

In rhinoplasty, the size of the nose becomes smaller or larger, and cartilage removed from different parts of the body can be used to increase the tissue. Most of the time, cartilage is removed from the septum. Occasionally cartilage is removed from the ear or ribs.

Step 4 – The correction of the deviation of the nasal septum

At this stage, the nasal septum is flattened and the protrusions inside the nose that interfere with breathing are corrected.

Step 5 - Close the cut

When the nose reaches the desired shape, a cover tissue is placed on the nose and the incision is sutured. It may also change its size by cutting into the wall of the nostrils.

Step 6 - See the results

During the first week after surgery, internal or external splints are used to support the nasal tissue while the nasal tissues are healing. However, the healing of the inflammation does not take more than a few weeks, until the shape of the nose changes one year after the operation, during which time the final result is achieved, and in the morning it may appear larger and swollen due to more inflammation. It is generally during the first year after surgery.

Note: Rhinoplasty requires examination of the nasal structures and the airflow path inside the nose to correct the airways. In some cases, the deviation of the nasal septum causes this problem, which is solved by smoothing it.

Important points about the safety and complications of rhinoplasty

The decision to have rhinoplasty is a personal decision, and one must personally consider the potential benefits and risks of achieving one's goals and accept the potential risks. The surgeon and the medical team will give you enough information about the possible risks. You will be asked to sign a form stating the risks and complications of the operation and you have accepted it.

Complications include

Rupture of the superficial vessels of the nose
Infection
Delay in wound healing
Dangers of anesthesia
Bleeding (hematoma)
Asymmetry of the nose
Cardiopulmonary complications may be seen in long-term use and may be associated with venous thrombosis.
Numbness in the skin

Rupture of the middle septum of the nose, which of course is rare and can sometimes be removed with surgery, and sometimes not.

It is important to ask your plastic surgeon about the surgical procedure. You may be anxious, which is normal, but this should not stop you from asking your questions.

When you went home

If you experience shortness of breath, chest pain, or irregular heartbeat, go to the hospital because you may need to be hospitalized. Medicine and surgery is not an accurately predictable science, and although good results are expected, they may not achieve the desired result and require reoperation. This can be a minor surgery.

Pay attention

Listening to the doctor's instructions is the most important factor in achieving the desired goal in surgery. The surgical incision should not be subjected to pressure, stretching, scratching, or movement during the healing process. Your doctor will give you the necessary instructions.

Recovery period

After surgery, a splint or tube or bandage may be placed inside your nose, and a splint or bandage may be placed over your nose to protect your nose.

In non-emergency applications, the cost is significant. The cost of rhinoplasty can vary depending on the surgeon's skill and where you perform the surgery, so ask your surgeon about costs.

When rhinoplasty is performed to improve the airways, it is a reconstructive procedure and may be covered by insurance, which of course requires examination and confirmation of the cause. Consult your doctor and insurance in this case.

Post-rhinoplasty care:

Up to three days of cold compresses on the face and graft site (if any), then warm compresses accelerate the absorption of bruises and swelling. The pain lasts for a maximum of three days and then decreases, during which the analgesic will respond.
To reduce facial swelling when resting, keep your head higher than usual.
Avoid strenuous activity and bending for a few weeks.
If the above is not observed, there is a possibility of prolonged swelling.
Mild bruising is normal.
Fresh pineapple (not compote) contains bromelain and vitamin C, which is effective in accelerating the absorption of bruises and inflammation, and the more it is consumed, the better.
Consumption of fruits, fish, shrimp and omega 3 is highly recommended.
Bloody discharge is normal for several days. But in case of obvious and significant bleeding, be informed (this is possible up to two weeks after the operation).
A visit on the third day is necessary to remove the tampon (if any) and the nasal cannula.
A first week visit is required to remove the splint. Also, a slight correction may be needed in this session.
In the open method, the swelling of the tip of the nose is longer than in the closed method, and in both methods, corticosteroids may be needed.
The final results of the operation will be obtained after six months to two years, so the earliest time to decide on a possible corrective operation is six months after the operation (except in rare cases).
The amount of swelling may increase for up to three days, which is normal, however, in case of significant pain, contact your doctor.
The amount of swelling is variable and is higher in the morning, which is minimized by following the principles of case number 2.
Reduce salt consumption to a minimum for 10 days.
Take your medication to the end.
Smoking for several weeks after surgery delays recovery and increases complications.
